Congestive Heart Failure Collaborative
The long-term goal of the Congestive Heart Failure (CHF) Collaborative is to build an infrastructure for chronic disease management in BC that will maximize the length and quality of life for patients with chronic disease and satisfy both patient and caregiver needs while maintaining or decreasing the cost of care. This will be achieved by implementing a system-wide model that focuses on improving interactions between patients and providers.
The Healthy Heart Society, the Collaborative Steering Committee, the Health Authorities and the Ministry of Health assisted participating physicians to achieve the goals and the aims outlined in the Congestive Heart Failure Collaborative Charter (PDF).
This video chronicles the experience of Dr. Neil Hilliard, a general practitioner from Chilliwack. It shows how one physician practice can dramatically improve patient outcomes and enhance professional satisfaction over a 12-month period by implementing a proactive planned approach to care for patients with chronic illness.
