The Fraser Health Authority Primary Healthcare Renewal Plan was designed to provide comprehensive, coordinated services to patients and their families. As part of the primary healthcare renewal initiatives, Fraser Health created a model of Enhanced Family Practices (EFP) to run as a pilot project until March 2006. The mission of the EFP was to improve personal and population health outcomes through the provision of high quality primary healthcare. Actions taken to achieve this mission included health promotion, continuous coordinated care and performance improvements.
