Practice Support Program | How's Your Health
The General Practice Services Committee (GPSC) is testing the value of supporting BC's physicians and patients to use the "How's Your Health?" interactive web tool through the Practice Support Program (PSP).
This initiative aims to provide a method for the patient experience to be included in primary care practice redesign. From March 2010 - May 2010 we will work with patients and primary care providers across BC to value-test the How's Your Health (HYH) suite of tools. When these tools are used over time, they can improve patient-centred physician office efficiency, access, quality of care, self management supports and patient provider experience. A customized virtual collaborative called the Institute for the Ideal Medical Home (IIMH) will be used with 20-30 teams in the province. From May to September 2010 work will continue with a subset of physician practices to understand sustainability.
