A Family of Technologies to Support the Health of the Population and Patient-Centered, Collaborative Care
Dr. John Wasson - June 21st, 2009
A key aspect of Patients as Partners is facilitating patient's involvement in their own healthcare. This information can be used to support self care and shared decision-making, encourage better clinical care and understand the patient's experience of care. HowsYourHealth is one method developed to meet these needs. View the web conference with the developer of HowsYourHealth, John Wasson MD.
Objectives of web conference:
- Distinguishing Measures for Action and Measures for Comparison
- Understanding HowsYourHealth (HYH): An Assessment and Feedback Instrument for Real Time Action and Comparison
- At the Population Level
- At the Practice/Employer Level
- At the Patient Level
- Controlled Trial Results
-
