Patient Self-Management - Learning Outcomes

Group learning sessions are available in most local communities. They are designed to help Clinical practices support patient self-management, as well as provide participants with opportunities to network with colleagues.

Each learning session is followed by an action period, in which session participants can experiment with the knowledge obtained in the session. Support is available from the regional Practice Support Teams for any issues that may arise during this time, such as help identify barriers, track progress, exchange information, address sustainability, and motivate participants.

After completing Enabling Patient Self-Management, a practice will be able to:

  • Educate patients on healthy lifestyles and behaviours
  • Help patients set goals to facilitate self-directed behavioural change
  • Provide patient support and assistance in solving daily problems
  • Conduct consistent patient evaluation and follow up of self-management plans
