Practice Self-Assessment
The Practice Self-Assessment Questionnaire is a simple first step for identifying current needs of a Clinical practice and areas for change or improvement. The questionnaire can help identify priorities for making changes and improvement and explores different ways of working that may enhance the effectiveness of a practice.

The Practice Support Program (PSP) has developed training, support and resources to meet many of these needs. The support includes funding to help physicians and their medical office assistant(s) implement changes identified as priorities for their practice. Other resources and supports can also be provided based for individual practices based on feedback.
Potential Opportunities for Enhancing a Clinical Practice
- Clinical - Embed evidence-based best practices in routine care or improve access to and communication with local specialists, other health care providers, or health authority-based services.
- Practice management - Develop a more effective on-call support network, or change the method of scheduling appointments to improve patient access without lengthening the work day.
- Information management and technology (IMIT) - Access and optimize available IMIT supports to increase the practice's capacity to electronically access data (e.g., lab results, medication profiles, chronic disease patient registries, and planned recall tools).

Accreditation
This program meets the accreditation criteria of the College of Family Physicians of Canada and the long form questionnaire has been accredited for 5.0 MAINPRO-C credits (effective until July 4, 2010).
