Chronic Disease Management
Chronic Disease Management workshops provide physicians, medical office assistants (MOAs), and other healthcare providers with the skills to implement planned care through the use of patient registries, flowsheets and an automated recall system (the CDM Toolkit).
The Chronic Disease Management “Bundle” is a series of training sessions specifically related to issues in CDM. The CDM Bundle contains 5 main components:
- Developing Patient Registries
- Implementing Planned Recall
- Adopting the CDM Toolkit
- Group Medical Visits
- Patient Self-Management

Learning Outcomes
Group learning sessions are available in most local communities. Each session is followed by an action period in which session participants test and implement changes in their own practice. Support is available from the regional Practice Support Teams to help identify barriers, track progress, exchange information, address sustainability and motivate participants.
Accreditation
This program meets the accreditation criteria of the College of Family Physicians of Canada and has been accredited for a maximum of 25.0 MAINPRO-C credits (effective until July 4, 2010) or alternatively 3.5 MAINPRO-M1 credits per 3.5 hour learning session.
