Physician Profile Analysis Report

The BC Ministry of Health report provides a simple means to get started with developing patient registers for chronic disease patients and helps physicians engage in planned, proactive care. It is free, secure, confidential and available by request to all practicing family physicians in BC. 

Request the report for an analysis of practice demographics to highlight your practice's chronic disease patients, billing patterns for chronic disease incentives and proxy measures for quality of care.

The current version of the Physician Profile Analysis Request Form (v2.0) includes practice profile information, plus registers for DM, CHF, CKD, COPD and HTN.

Note that patient registers are updated each fiscal year, once all incoming data for the year has been processed. This usually occurs sometime in July. Because of this, you may want to request a new report each year. To request a report:  

  1. Be sure to include your email address. Having an email account is MANDATORY as the report is sent as an encrypted CD via Canada Post and a 64 character password is sent via email.
  2. Note that the Ministry will notify your Health Authority's Practice Support Program/Regional Support Team to let them know that you have requested a report (they do not receive any information that is in the report itself). You may choose to opt out of having your HA notified by ticking the box on the registration form.
