Care Delivery Model Redesign Collaborative

Description

The Vancouver Island Health Authority is partnering with Fraser Health, Northern Health and Providence Health to implement a one year Structured Learning Collaborative (a framework developed by the Institute of Healthcare Improvement) that aims to “achieve care delivery that is responsive to the care needs and experiences of patients, reflects inter-professional practice, and is based on data and evidence”.

This approach brings together front line staff and leaders of 60 hospital units to learn how to make and sustain improvements on their units. The Collaborative is structured using targeted change areas, a single change methodology and common metrics and collaborative because all participants share and learn from each other.

Our Role

ImpactBC is providing the following services to VIHA for this initiative:

  • Coaching and facilitating a Structured Learning Collaborative process including the design, content development, change packages, learning sessions and action period support and faculty development
  • Support managers/leaders of the different service areas so they develop the capacity to lead and sustain improvement initiatives
  • Expertise in methods for quality improvement
  • Tools to help with implementation of changes and tracking of measures
  • Linkages/liaise with national & international experts on quality improvement projects
