Healthy Heart Group Medical Visits

Description

This project fell under a Contribution Agreement with the Public Health Agency of Canada - Canadian Diabetes Strategy. 

Phase I of the project, which ran from February 1 to June 30, 2010, supported 4 primary care practices and the Heart Program for Women at BC Women’s Hospital and Health Centre to provide cardiovascular disease (CVD) risk assessment group medical visits for patients with at or high risk for diabetes.

Phase II of the project which ran from July 1, 2010 to March 31, 2011 focused on improving the Heart Program for Women’s capacity to provide cardiovascular disease (CVD) risk assessments and self-management support for women with post-partum gestational diabetes.

 

Our Role

ImpactBC supported the Heart Program for Women to evaluate the effectiveness of group medical visits with and without the addition of a peer coaching component to improve outcomes and satisfaction in women with post-partum gestational diabetes by providing:

  • Project management support for five intake CVD risk assessment group medical visits, five 3-month follow-up group medical visits, and peer coaching offered through the Patient Voices Network
  • Resource development and production
  • Staff and Peer Coach training
