Mapping the Patient Journey

Supporting and building capacity for mapping the patient journey in integration communities to help guide the integration of primary and community care.

Project goal

Train the trainer in five Integrated Primary Care Communities (IPCC) to build capacity for patient journey mapping and to spread and sustain the knowledge across all integration work.

Objectives:

  • Provide training using a "Web in Action" format
  • Share and learn together across teams
  • Spread to other IPCC communities and projects
  • Develop and test a web based community of learning

Our role

An advisory committee has been formed to support the development and implementation of this work. A Web in Action format has been used: this is a series of three capacity building webinars with action periods during which learnings are tested. Coaching support and opportunities to share and learn together are provided. All participants are using the web based community of learning to access resources, ask questions, post their activities and discuss learnings with others six communities will have completed the mapping by June 2011, with additional teams completing by September 2011. A share, learn and spread congress is planned for September 2011. A 2nd Web in Action series will be offered in October.
