The Heart Manual

Description

The Heart Manual is an individualized home-based self-management program for people with coronary artery disease and related conditions. 

The Heart Manual may be used as a stand-alone home based program, or it can be integrated into existing hospital or community based programs (including physician’s offices), as either a primary or secondary prevention tool. 

It can be followed in the comfort and privacy of one’s own home, and may be facilitated by a trained health professional in a number of ways:

  • in person, one-on-one
  • in person, in a group setting
  • over the phone

The patient materials are menu-based and can be tailored to an individual’s needs. The Heart Manual (Coronary Artery Disease Edition) is suitable for people with the following conditions:

  • Angina
  • heart failure
  • angioplasty/stent
  • coronary artery bypass graft (CABG)
  • implantable cardiac defibrillator (ICD)
  • heart attack
  • diabetes
  • hypertension
  • metabolic syndrome
  • dyslipidemia
  • obesity
  • anxiety
  • depression
  • stress

Our Role

ImpactBC has negotiated to make the Heart Manual, originally developed in the UK, available in Canada.

ImpactBC provides the following services:

  • Train health professionals wishing to facilitate the program with patients
  • Provide (sell) Heart Manuals to Health Authorities for distribution / use with patients
  • Support health authorities / health providers to evaluate their use of the Heart Manual, using a quality-improvement approach
  • Provide additional self-management & goal-setting support to patients through the Patient Voices Network Peer Coaching program
