Pictures say more than words. Videotapes and photos put a human face on health system improvements.  

The Impact BC website features several videos that  highlight the role of various health providers, as well as specific issues that arise when improvements are implemented. They are appropriate for primary healthcare and chronic disease management coordinators to show in clinical practices, in training sessions, at conferences or for uploading to websites.

The Leading Edge: Chronic Disease Management in Massett (8:00 mins)

 The Leading Edge chronicles Dr. Vanita Lokanathan's experience with implementing the collaborative approach to managing care in a rural practice. Highlights use of the CDM Toolkit, recall systems, group visits, community engagement and the use of specialists in a team approach.

Making Big Changes

This video chronicles the experience of Dr. Neil Hilliard, a general practitioner from Chilliwack. It shows how one physician practice can dramatically improve patient outcomes and enhance professional satisfaction over a 12-month period by implementing a proactive planned approach to care for patients with chronic illness.

Patient Experience Video Series

The Patient Experience series includes 6 short videos that share a common message: a coordinated, integrated approach to primary healthcare throughout the province benefits both patients and providers. The videos highlight diverse patient experiences through short vignettes that illustrate the benefits of an integrated approach to primary care. The series includes:  

  • Nothing About Me Without Me (10:00)
  • Nothing About Me Without Me is the first video in this series. It provides an overview of key themes by including highlights from the shorter vignettes that capture assorted patient and provider experiences with integrated care, and explores how Integrated Health Networks (IHNs) enable patients with chronic conditions to become active and informed contributors to their health by working in partnership with their healthcare team.


  • Surinder Singh Chauhan: Coordinated Care in the Community (4:00)
  • Language barriers and cultural differences can often obstruct access to coordinated healthcare, leaving patients in the dark about their conditions. So how do we bridge that gap in communications to ensure that patients get the information they need to take charge of their health? Learn how a bypass surgery patient overcomes this obstacle by regularly attending a community heart program offered in Punjabi, his first language. Opening the door to a variety of health services and information, the program gives Surinder the knowledge and confidence he needs to take charge of his own health.


  • Susan Orchard: From Fragmented to Integrated Care (4:00)
  • Fragmented to Integrated Care addresses the integration of primary and mental healthcare and demonstrates how integrated care in a small, remote community can provide continuity and a coordinated approach to patient care. The video profiles Susan - a diabetes patient in the small northern town of Mackenzie who also struggles with obesity and schizophrenia - and shows how she works with her mental health clinician to make positive changes that improve both her physical and mental health.  


  • Bruce Bennett: An Activated Informed Patient (4:00)
  • An Activated Informed Patient demonstrates how patient action can have a positive impact on the health of a community. This video chronicles Bruce Bennett's story and shows how he takes responsibility for his own health and helps others do the same.


  • Allan Tapper: Listening to the Patient Voice (5:00)
  • Listening to the Patient Voice demonstrates the value of strong patient/provider relationship. The video profiles the story of Allan -  a participant in an Integrated Health Network (IHN)  who receives continuous, comprehensive care through the combined efforts of the family physician and a chronic disease management (CDM) nurse. The focus is on the special bond that develops between Allan and his CDM nurse and how this relationship helps him manage his chronic conditions.


  • Gene Pascuzzo: Mapping the Patient Journey (6:00)
  • Mapping the Patient Journey demonstrates the importance of patient involvement in health system re-design. The video profiles Gene Pascuzzo and shows how he helped develop a strategy for using Group Medical Visits (GMV) as a vehicle to improve patient care.

Personal Health File (5:51 mins)

Personal Health File demonstrates how patient self-management programs can benefit from the use of personal health files and explores how the use of these patient-maintained print versions of an electronic medical record can inspire patients to take charge of looking after their own health.

Supporting Patient Self-Management (6:00) 

Supporting Patient Self-Management demonstrates the "Three Questions" technique that physicians can use to help patients create an action plan to support self-management of their chronic condition(s). The video promotes and encourages patient self-management through Group Medical Visits (GMV), where patients openly discuss the self-management process, ask questions related to their chronic conditions and get support from their peers.
